We appreciate your interest in joining our Therapeutic Riding Program! Please download the forms of our Rider Application Packet below.  Once we receive these forms, the rider is placed on our waiting list, currently about 24 months long.

Two forms, the Contraindications Form and the Physician Consent Form, must be completed by the rider’s physician.  The completed packet can be dropped off at the Adoptions main desk, in an enveloped marked to the attention of Program Manager Alicia Roe, or they can mailed.  Our mailing address is:

Due to confidential nature of the information requested on the forms, we ask that they not be faxed. Also, since a physical signature is required, they may not be e-mailed.
